Vaccination Clinic at Skinner! Nov. 18 & Dec. 16

The FDA recently approved the 2-dose Pfizer vaccine for children age 5-11. Getting your child vaccinated will protect them from COVID-19 and also help slow the spread of this virus in our community. We are partnering with Denver Health to host a FREE after-school vaccine clinic that is open to any person age 5 and older. The clinic at Skinner will be from 4-7PM on Thursday, November 18th (1st Dose) and Thursday, December 16th (2nd dose)

For each COVID Vaccine Clinic, there will be 300 pediatric doses (ages 5-11) and 90 adults (12+ doses). If children are under 18, parental consent will be required.  Children under the age of 11 must have a parent present at the time of vaccination.  Click below for links to consent for treatment form:

**Pzifer adult/booster doses will be available.
**Patients who receive their first dose at the clinic on 11/18 at Skinner will be auto-scheduled for 2nd dose on 12/16. If they want to change that they can call and reschedule. If you do not have a MyChart account, you will need to create one. Please use your personal email to create your MyChart account. Once in MyChart you will be able to schedule a vaccine appointment.
